This is for any amendments to the Financial Statement or Key Information sheets plus any extra information you think may be useful. This should be returned to us.
This will be sent to your creditors, the companies you owe money to, and informs them that you have agreed that we can act on your behalf. This needs to be signed by you (and anyone whose name is on the account) and returned to us.
This is an instruction to your bank to make payments to our account as we will be paying your creditors from that money. We have already completed our details, you just need to add your bank details and sign it. If there is another signatory on your account, they need to sign it too. This needs to be returned to us.
Please send us statements from all your creditors, anything you have.
We also need evidence of your income – a payslip from an employer, details of benefits etc.
Finally we need evidence of identity such as a passport or driving licence.
This is our summary of the information you gave us over the phone and explains why we have recommended a plan for you. If we have misunderstood any details, please write the correct information on the Additional Information sheet. You keep the Key Information sheet.
This shows your income and expenditure and explains what payments will be made to your creditors. Please check it. If anything is wrong, please write the correct information on the Additional Information sheet. You keep the Financial Statement.
